As soon as you are a patient of our practice, you can conveniently pre-order prescriptions or transfers via the online service.<\/p>\n For a successful treatment by your family doctor, it is important that we know your medical history. For this purpose, we ask you to print out our questionnaire<\/a> for new patients before your first visit and to fill it out at home.<\/p>\n Please bring with you to your first appointment:<\/strong><\/p>\n Information about the treatment of minors<\/a><\/p>\n If you are unable to come to the doctor\u00b4s practice with your child for treatment, we can offer the following two options with a power of attorney:<\/p>\n Please bring the child's insurance card, the child's vaccination certificate and an official photo ID from the representative with you to the appointments. Please note that preventive medical examinations can only be carried out in the presence of a legal guardian.
